Admission and Registration..........

The Children’s Workshop Early Learning Centers offer programs for children age 1 year through 12 years.

The following requirements must be met, before a child begins:

1. Pre admission consultation between the parents and the director
2. Registration and Authorization Card completed, signed and returned
3. IEF completed, signed and returned
4. Child’s Health Examination completed, signed by physician and returned
5. Immunization Form completed, signed by physician and returned
6. Financial Agreement signed and returned
7. Illness and Absence Policy signed and returned
8. Registration Fee paid
9. Tuition for first week paid
10. Contract for services, if applicable
